Nov. 22--Editor's note: These figures are derived from inspections conducted by the Lewis County Public Health Department's Food Safety Program.

Red violations are those most likely to cause foodborne illness and must be corrected at the time of inspection. Blue violations relate to overall cleanliness and operational conditions and must be corrected by established deadlines or by the next routine inspection.

Any establishment receiving 40 red points or any red point item repeated within an 18 month period is considered a high risk and must be reinspected. An establishment that receives 75 red points or 100 total points (red and blue) on a routine inspection or 40 red points on a repeat inspection will have their food establishment permit suspended.

Freeway Mart, Centralia (0/5/5)

An indirect drain is required on the three-compartment sink. Please correct within two months. Left handout. (5 blue)

Inspection: Nov. 6

Gee Cee's Inc., Toledo (15/0/15)

There are four large pieces of raw, slaughtered game animal being stored separately in a walk-in freezer. Per staff, these are for the personal use of the owner and are not being processed or cooked in the establishment. This practice is not acceptable, however, and the meat must be removed immediately. Do not utilize this establishment for game animals in the future. (15 red)

The Restaurant, Chehalis (10/0/10)

Prep cooler was not keeping proper temperatures, and potentially hazardous foods were found between 46.6 and 52 F. All PHFs must be kept at 41 F. or below at all times. These were either discarded or moved to another unit to cool, based on time and temperature. This is a repeat violation. (10 red)

Inspection: Nov. 5

Travelodge, Centralia (5/5/10)

Food worker in charge did not have a valid food worker card. Please ensure that all food workers, even new employees, have valid cards. Please correct within one week. This a repeat violation. (5 red)

There were no test strips available to test sanitizing solution. This is a repeat violation. Please correct within one week. (5 blue)

Due to repeated violation for not having test strips, a $100 fine must be paid to the Health Department within 10 days.

Brownstone Coffee Lounge, Centralia (10/0/10)

Potentially hazardous foods were found cold holding between 42.4 and 53.7 F. and must be held at 41 F. or below at all times. This is a repeat violation. Yogurt was discarded and refrigerators will be readjusted. (10 red)

Centralia Eagles, Centralia (25/0/25)

Bare-hand contact was noted with hamburger buns. These were discarded. Bare-hand contact is not allowed with ready-to-eat foods. (25 red)

Newaukum Valley Golf Course, Chehalis (5/0/5)

Food cold holding temperature was too high in the "makeit" refrigerator. This was pulled out farther from the wall by the operator to allow for proper clearance and will be retested for temperature later today. (5 red)

Inspection: Nov. 12

Ocean Sky, Chehalis (25/3/28)

Several potentially hazardous foods were found cooling improperly. Please cool all PHFs in depths no more than 2 inches thick, uncovered in the walk-in until 41 F. or below. These were transferred to shallow pans to cool. (25 red)

Entire kitchen needs to be better cleaned, including floor, counters, fan, refrigerators, etc. This is a repeat violation. (3 blue)

Due to the third cleanliness violation, a $100 fee must be paid to the Health Department within 10 days.

Inspection: Nov. 10

Quality Cafe & Pizza, Chehalis (5/10/15)

There was no digital thermometer available for restaurant use. Please provide before opening by tomorrow. This is a repeat violation. (5 red)

There was no bleach bucket available. This was corrected. This is a repeat violation. (5 blue)

Dishwasher was not sanitizing. This was corrected. This is a repeat violation. (5 blue)

Due to repeated blue point violations, a $200 fee is due to the Health Department within 10 days.
